Potential Consumers that need personal care

1. Must have FULL Medicaid Benefits (Turquoise Care) with one of the following insurances:

*Presbyterian

*Blue Cross Blue Shield

*Molina

*United Health Care

2. Must be 21 Years Old or Older

3. Must have a mental/physical disability

4. Your mental/physical disability must enable you to need assistance in 2 or more areas such as:

*Hygiene & Grooming

*Meal Preparation

*Supportive Mobility Assistance

*Light House Keeping

*Companionship

*Medication Reminders

*Running Errands (Medication Pick-Up, Store, and Charity Food Box Events)
